1.0 Introduction


Online survey


Online survey module provides function to manage the information that related to questionnaire. This module enable users to manage the features such as Online Survey Template, Manage Question Category, Add New Survey and Publish/Unpublished Online Survey.



2.0 Setting


Before performing step by step process or normal functions under Online Survey Module, user must complete the setting. This is to ensure smooth process in running the Online Survey Module.



The setting must be completed before proceed to manage the online survey. Two types of setting which are Answer Template and Question Category.




2.1 Answer Template

Click on Answer Template Link to add the Answer Template for online survey as below.



Complete all the information of the template as below.


1. Template Name

2. Description

3. Answer Choice 

-For example, the answer to be choose is True or False. Kindly fill 'True' in the field and click on Add More button to add 'False'.

4. User is able to copy the template from Template Answer

5. Once done, click on Save button.




2.2 Question Category




User need to add the question category. Example of the category is Food and Beverage. User need to fill up the code, name,description and status.



3.0 Manage Online Survey


Once the setting is complete, user is able to manage the online survey. User is able to add new survey, update and delete the online survey.


3.1 Add New Survey


Click on Add New Survey link to add the survey as below.




User need to fill up all the information as below.



1. Survey Name

2. Description

3. Status - Active/Inactive

4. Survey Type - Normal Survey, Lecturer Evaluation Survey & Subject Evaluation Survey

5. Once done, click on Submit button


What is Normal Survey?     


Normal survey is a general survey. For example Transportation Survey, Internet Access Survey, Library Facilities Survey and etc.


What is Lecturer Evaluation Survey


Lecturer Evaluation Survey is an evaluation from student to evaluate their lecturer according to subject taken.


What is Subject Evaluation Survey


Subject Evaluation Survey is an evaluation from student to evaluate their syllabus of subject.


3.2 Manage Question


User need to filter the filtering options, only then the list of online survey will appear in the list.




1. Fill up Survey Name

2. Select Survey Category


All the list will appear s below once user click on Search button. User is able to update, delete and manage question.



Legend


Blue : User still can edit the question, delete and update due to not publish yet to a student.

Pink : User is not allowed to edit, delete or manage the question due to had published to student.


1. Update - To update the online survey.

2. Delete - To delete the online survey

3. Manage Question - User is able to manage the question (Refer attachment below).






1. Select Question Type - Each type of  question has a different structure (eg : dropdown menu, multiple choice, rating and etc).

2. Copy from other survey - user is able to copy the question from other survey




4.0 Publish/Unpublished Online Survey





4.1 Publish Online Survey



User is able to publish the online survey to a students. User need to click on Publish New Survey link as below.




User need to select the criteria options, and list of student will appear once user click on Filter button.



1. Select Faculty

2. Select Program

3. Select Academic Session

4. Select Survey

5. Click on Filter button


List of student will appear in student list, and user need to fill up the start date,due date and tick on student name or tick all the list of students.





1. Start Date - Student can start to complete the online survey

2. Due Date - Due date for students to complete the online survey

3. Tick all the list of students to be published

4. Click on Submit

5. The name of student will disable if student not register the subject (Subject Evaluation Survey) and if student is not registered into the section (Lecturer Evaluation Survey).


Once survey is being published, the list of student will appear as below (refer attachment), if students has answered the question, in the action will appear link 'View'.






If user click on 'View', user can see the survey answer that has been done by student. Refer attachment below.





4.2 Unpublished Online Survey


To unpublished the online survey that has published, user need to filter the criteria options and unpublished the online survey as below.



List of student will appear as below. If student has answered the question, the student name will disable to unpublished. You may refer the sample of student as below (Amin Bin Mohamad), his name is ticked as disable.



1. Tick on student name.

2. Click on Submit button.


Once user unpublished the online survey, the notification in student portal will disappear. 




5.0 Student Portal

Once user published the online survey, notification will appear in student portal. Student need to complete the online survey.





1. Student need to click the notification to complete the online survey and need to answer each question as below.





- Normal survey - student can proceed to complete the online survey without confirmation.

- Subject survey - student need to confirm first regarding the subject taken before proceed to complete the online survey.

- Lecturer evaluation - student need to confirm first on their lecturer name before proceed to complete the online survey.



6.0 Reports


We have provide Individual Responses Report. Staff can view the responses of student.




Click on Individual Responses Report.



1. Search Student Name

2. Select Faculty

3. Select Program

4. Select Academic Session

5. Select Survey Type

6. Submitted Date

7 Click Generate Button



Below is the sample of Lecturer Evaluation Report  that has been done by student. Staff can export the report  to XLS format and print.
